Job OverviewThis role is responsible for providing physiotherapy consultation, assessment, and interventions for various client groups across Southern Ontario and the Capital region.
You will work within a quality improvement framework that reflects our vision and mission of services.
To promote good public relations with clients, customers, and our team, volunteers, and associates.
Key Responsibilities- Assess clients using interviews, standardized, and informal techniques in accordance with professional practice guidelines and standards.
- Implement client interventions based on assessment information, client input, and integrated care plans.
- Evaluate services provided through outcome measures, ensuring best practices, evidence-based care, and documentation.
- Attend meetings to report on assessment findings and discuss intervention plans, progress reports, and discharges as required.
- Ensure coordinated care delivery through ongoing communication with the client's team regarding plans and schedules.
- Document all assessment findings, treatment plans, progress, and discharge summaries as needed.
- Prepare records of client care and related activities as required by VHA, the Ministry of Health, and other agencies.
- Baccalaureate or Masters degree in Physiotherapy from a recognized school.
- Registered with the College of Physiotherapists of Ontario.
- Minimum 2 years clinical experience, preferably in the community.
- Specialized courses supporting areas of clinical expertise.
- Clinical expertise to provide various physiotherapy interventions.
- Familiarity with community care requirements and resources available.
- Excellent consultation and teaching skills, utilizing an evidence base to clinical practice.
