Nurse Practitioner

Job Summary:
Closing the Gap Healthcare Group is seeking a skilled Nurse Practitioner to join our team in a community care setting. As a key member of our healthcare team, you will be responsible for providing high-quality nursing care to clients in their homes and communities.
Key Responsibilities:
- Provide comprehensive nursing care to clients, including assessment, planning, implementation, and evaluation of care.
- Develop and implement individualized care plans that address the unique needs and goals of each client.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Document client information and updates on their treatment plan in a timely and accurate manner.
- Participate in ongoing education and professional development to stay current with best practices and industry developments.
Requirements:
- Graduate from a recognized accredited School of Nursing.
- Registration in good standing with the College of Nurses of Ontario as an RN (Registered Nurse) or RPN (Registered Practical Nurse).
- Experience with home care and community care is preferred.
- Excellent documentation and time management skills.
- Professional membership preferred (CNA, RNAO, WeRPN).
- Advanced certifications are an asset (NSWOCC, IIWCC, SWAN, CVAA, etc.).
