Job Summary

Prairie Mountain Health is seeking a skilled Home Care Case Coordinator to join our team. As a key member of our multidisciplinary team, you will be responsible for providing a collaborative, client-driven process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet the client's health and human service needs.

Key Responsibilities

  • Receive and review referral information, prioritize, and determine eligibility for Home Care services or re-direct referrals appropriately.
  • Conduct comprehensive in-home client assessments to determine eligibility and care needs related to Home Care and/or Personal Care Home admission or alternative.
  • Admit eligible cases to Home Care or re-direct appropriately.
  • Complete home safety risk assessments to ensure a safe work environment.
  • Work collaboratively with clients and families to provide holistic, client-centered care, including timely and effective communication with other healthcare professionals during care transitions.

Care Planning & Case Coordination

  • Analyze data from in-home assessments, identify, and prioritize client needs and strengths.
  • Plan and implement client-centered goals, care plans, and recommendations through clinical analysis of assessment findings.
  • Participate in discharge planning and refer or facilitate referrals to other professionals or agencies as required.
  • Manage caseload demands effectively and plan and organize own work schedule.

Monitoring and Evaluation

  • Manage client caseloads, including monitoring, evaluation, reassessment, and adjustment of care plans.
  • Participate in case reviews and conferences as assigned and submit regular reports as required.
  • Communicate pertinent client information and needs to managers in a timely manner.

Program Planning and Administration

  • Participate in interpreting services and resources provided by Home Care to the public and other agencies.
  • Establish connections and maintain relationships with local healthcare services and the informal community resource network.
  • Participate in ongoing quality management through the development, implementation, and evaluation of services.

Team Collaboration

  • Provide ongoing information and guidance to other involved professionals and caregivers within the team.
  • Provide consultation and information to families or other client care providers who will be administering and monitoring client care plans.
  • Share client care plans and pertinent information with stakeholders to support a collaborative approach to client care.

Professional Development

  • Identify own educational needs and develop an educational pathway to meet those needs.
  • Participate in regular performance evaluations and Continuing Competency programs as per professional licensing bodies.
