Job Summary

Prairie Mountain Health is seeking a skilled Home Care Case Coordinator to join our team. As a key member of our multidisciplinary team, you will be responsible for providing a collaborative, client-driven process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet the client's health and human service needs.

Key Responsibilities

  • Receive and review referral information, prioritize, and determine eligibility for Home Care services or re-direct referrals appropriately
  • Conduct comprehensive in-home client assessments to determine eligibility and care needs related to Home Care and/or Personal Care Home admission or alternative
  • Admit eligible cases to Home Care or re-direct appropriately
  • Complete home safety risk assessments to ensure a safe work environment
  • Work collaboratively with clients and families to provide holistic, client-centered care

Care Planning & Case Coordination

  • Analyze data from in-home assessments, identify, and prioritize needs and strengths
  • Plan and implement client-centered goals, care plans, and recommendations through clinical analysis of assessment findings
  • Participate in discharge planning
  • Refer or facilitate referrals to other professionals or agencies as required

Monitoring and Evaluation

  • Manage client caseloads, including monitoring, evaluation, reassessment, and adjustment of care plans
  • Participate in case reviews and conferences as assigned
  • Submit regular reports as required and notify managers of critical situations that may impact client health and safety

Program Planning and Administration

  • Participate in interpreting services and resources provided by Home Care to the public and/or other agencies
  • Establish connections and maintain relationships with local healthcare services and the informal community resource network

Team Collaboration

  • Provide ongoing information and guidance to other involved professionals and caregivers within the team
  • Provide consultation and information to families or other client care providers

Professional Development

  • Identify educational needs and develop an educational pathway to meet those needs
  • Participate in regular performance evaluations

Requirements

  • Valid professional license or certification
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
