Wellness Nurse RN

At Sunrise Senior Living, we are seeking a compassionate and skilled Wellness Nurse RN to join our team. As a Wellness Nurse RN, you will play a vital role in ensuring the overall well-being of our residents.
Responsibilities:- Conduct regular wellness visits for all residents to assess their health and provide personalized care.
- Develop and implement individualized care plans to meet the unique needs of each resident.
- Collaborate with the Resident Care Director and other healthcare professionals to ensure seamless care delivery.
- Monitor and report any changes in resident health status to the Resident Care Director and other relevant parties.
- Provide education and support to residents and their families on health-related topics and care management.
- Participate in quality improvement initiatives to enhance the overall quality of care and services provided.
- Graduate of an accredited nursing program.
- Current state license as a Registered Nurse (RN).
- Minimum of one (1) year experience in a healthcare setting, preferably in senior living or long-term care.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
- Strong problem-solving and critical thinking skills.
