Dietitian, Transitional Care Specialist

About the Role
We are seeking a skilled Dietitian to join our team at the West Bedford Transitional Care Centre in Bedford, Nova Scotia. As a Dietitian, you will provide clinical dietetic services to residents as part of the Interdisciplinary Team.
Key Responsibilities
- Assist in the development of menus in accordance with therapeutic nutrition guidelines.
- Assess clients' nutritional status on admission and at least semi-annually; review monthly weights and follow up as required.
- Complete texture modification assessments on referral; participate as a member of the interdisciplinary team.
- Address clients' individual dietary concerns.
- Monitor use of nutritional supplements using the "Shannex Nutritional Supplement Tools" and make adjustments as necessary.
- Develop and establish policies and objectives consistent with those of the organization and department to ensure achievement of clinical goals.
- Manage expenditures and work to ensure compliance with approved budget.
- Collect and enter appropriate data into Computrition software to ensure resident diet types and any likes, dislikes and allergies are accurately recorded.
- Participate as a member of in-house interdisciplinary clinically focused committees.
- Communicate client nutrition care plans to clients/family/staff as appropriate.
- Monitor and document progress of residents as high nutritional risk utilizing the MDS Rugs RAI 2.0 documentation system.
- Complete Quality Management Reports as required.
- Communicate with other Dietitians to assess client information prior to admission both to and from the facility.
- Oversee and plan special nutrition needs for special diets, allergies and disease conditions.
- Promote a safety culture by continuously working safely including but not limited to identifying and reporting hazards, following safe work practices and procedures, wearing personal protective equipment, supporting others to work safely and completing safety education.
- Ensure client safety by following and applying all standard operational procedures, safe work practices and training provided.
