We are seeking a skilled Licensed Practical Nurse (LPN) to join our multidisciplinary team at Horizon Health Network. As a key member of our rehabilitation services team, you will provide holistic care to individuals and groups of patients, working to the full extent of the LPN scope of practice.
Key Responsibilities- Participate in assessment, planning, implementation, and evaluation measures to meet patient needs and enhance outcomes.
- Aid patients/families in developing their care plan and in recovery planning.
- Act as a resource and advocate for patient and family-centered collaborative care.
- Collaborate with other team members to provide comprehensive care.
- Minimum of 1 year of recent and direct experience in addiction and mental health, or an equivalent combination of experience and education.
- Active registration with the LPN Association of New Brunswick (ANBLPN).
- Current BLS, Non-Violent Crisis Intervention, ASIST, and Motivational Interviewing certifications.
- High level of interpersonal skills and ability to work well within a team.
- Demonstrated awareness of assessment tools and treatment modalities.
- Analytical thinking.
