About the Role

As an Occupational Therapist with CBI Home Health, you will have the opportunity to work with diverse clients, addressing their unique needs and goals. You will be responsible for assessing clients, developing personalized care plans, and delivering targeted interventions to improve their functional abilities and quality of life. Your expertise will play a vital role in enabling clients to maintain their independence and live fulfilling lives in their own homes.

Responsibilities

* Assess clients and develop comprehensive care plans
* Deliver evidence-based occupational therapy interventions to improve client function and quality of life
* Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals
* Communicate effectively with clients, families, and healthcare teams to ensure seamless care coordination
* Document client progress and outcomes

Requirements

* Current registration with the College of Occupational Therapists of Ontario
* Strong understanding of person-centered care principles and best practices in occupational therapy
* Excellent communication and interpersonal skills
* Ability to work independently and as part of a team
* Strong problem-solving and critical thinking skills
* Familiarity with electronic medical records and other healthcare technologies
