Nurse Coordinator

Job Summary:
We are seeking a skilled Registered Nurse to join our team as a Transition Coordinator at the Royal Alexandra Hospital. As a key member of our multidisciplinary team, you will play a critical role in assessing patients' needs, developing and implementing care plans, and coordinating services across the continuum of care.
Key Responsibilities:
- Conduct comprehensive electronic needs assessments to determine urgency and priority
- Develop and implement appropriate service plans to meet patients' needs
- Coordinate service delivery across programs and settings
- Liaise with Continuing Care staff and other community resources
- Provide information and support to clients, families, and caregivers
Requirements:
- Completion of an accredited nursing education program
- Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA)
- Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP)
- A minimum of five years facility-based nursing experience, including geriatric experience
- A minimum of two years' experience in community nursing
Preferred Qualifications:
- Knowledge of computer applications such as Microsoft Word, Pathways, RAI Tools, Meditech, and e-People
- Active Transition Coordinators with up-to-date certification may receive preference
