Bonaventure Support Services is seeking a skilled Community Housing Health Care Assistant to join our team in the Parksville area. As a key member of our team, you will provide support to individuals with diverse abilities, promoting their dignity, independence, and individuality. Your role will involve providing personal care, operating an adapted wheelchair-accessible van, and collaborating with healthcare professionals to ensure the best possible outcomes for our clients.
Responsibilities:
- Provide personal care and support to individuals with diverse abilities, including assistance with daily living activities, medication management, and behavioral support.
- Operate an adapted wheelchair-accessible van to transport clients to appointments and activities.
- Collaborate with healthcare professionals, including doctors, nurses, and therapists, to develop and implement individualized care plans.
- Assist clients with activities of daily living, including bathing, dressing, and grooming.
- Monitor and maintain accurate records of client progress and medication administration.
- Provide crisis intervention and risk management strategies to support clients in times of need.
- Participate in team meetings and contribute to the development of care plans and strategies.
Qualifications:
- Preferred formal education (Diploma or Degree, CSW, RCA, SIVA, etc.) and/or work experience with persons with diverse abilities and/or other relevant experience.
- Excellent interpersonal behavior and communication skills.
- The desire and ability to work with persons with diverse abilities and demonstrate personal suitability to promote dignity, independence, and individuality.
- Ability to work independently in a high-stress environment.
- Must be in good health with the physical ability to carry out the duties of the position and shift responsibilities.
Required Documentation:
- Valid BC Class 4 Driver's License - or willingness to obtain one within 3 months of employment.
- RCMP Criminal Record Check (Schedule A - Employer will file).
- MCFD Hub Screening (Employer will file).
- First Aid with CPR Certificate.
- Food Safe Level 1 Certificate.
- Immunization Records or Medical Exemption.
- Signed Bonaventure Hire Package.
- 3 References - (One must be a most recent employer, second and third must be able to speak to your ability to work with vulnerable youths).
- Physician's Declaration.
- TB Screen.
