We are seeking a highly skilled and compassionate Geriatric Case Manager to join our team at Geriatric Psychiatry Community Services of Ottawa. As a key member of our community-based mental health program, you will play a vital role in providing comprehensive assessment, treatment, and follow-up services to older adults with mental health problems and/or dementia.

About the Role

  • Assessment and Treatment
    • Conduct thorough, multi-dimensional psychosocial assessments of clients in their homes and develop personalized treatment plans.
    • Deliver evidence-based interventions to support clients in managing mental health symptoms, reducing behavioral expressions, and enhancing caregiver understanding and support.
  • Collaboration and Communication
    • Work closely with geriatric psychiatrists, community partners, and healthcare agencies to ensure seamless care coordination and support.
    • Communicate effectively with clients, families, and healthcare professionals to understand client needs, implement treatment plans, and balance risk versus autonomy.
  • Qualifications and Requirements
    • Master's degree in Social Work, Occupational Therapy, or a related field.
    • At least three years of experience in geriatric psychiatry or a related field.
    • Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) or a related professional college.
    • Comprehensive knowledge of psychiatric conditions affecting older adults, community resources, and evidence-based practices.
    • Excellent written and verbal communication skills, with the ability to work effectively in a team environment.
