Compassionate Home Care Professional

VON Canada is seeking a dedicated and compassionate Home Care Professional to join our team. In this role, you will provide direct hands-on care to patients in their homes under the direction of a Directing Registered Nurse (DRN).
About the Role:You will work as part of a multidisciplinary team to provide personalized support to clients with varying needs, including those requiring palliative and pediatric care. This is an excellent opportunity to make a meaningful difference in the lives of others while developing your skills and expertise.
Key Responsibilities:- Observe and document clinical data in the patient's electronic medical record as per VON guidelines.
- Provide personal care to support activities of daily living and medication administration.
- Collaborate with the DRN to develop and modify a patient's care plan based on observed outcomes.
- Communicate effectively with clients, family members, and healthcare professionals to ensure a positive patient experience.
- Possess Personal Support Worker (PSW) certification or equivalent.
- Have one to two years' experience as a PSW in Palliative/Pediatric or acute care (hospital).
- Hold current CPR and First Aid certification.
- Demonstrate knowledge of fundamentals of Palliative Care- Level I.
This role offers a competitive salary of $28.50-$35.00 per hour, depending on location and experience. You will also have access to ongoing training and development opportunities to enhance your skills and career prospects.
