Closing the Gap Healthcare is seeking a highly skilled and experienced Home Care Specialist to join our team. As a key member of our Integrated Care Solutions team, you will be responsible for overseeing service provider personnel and ensuring compliance with policies on client safety, risk reduction, and staff safety.

Key Responsibilities:
  • Oversee service provider personnel and ensure compliance with policies on client safety, risk reduction, and staff safety.
  • Assess, plan, collaborate with the client, family, and other members of the health care team to establish and/or update the care plan and implement and evaluate client care with every client encounter.
  • Use a variety of nursing skills such as IV antibiotic administration, IV Cannulation, Chemo disconnects, CVAD care, and PORT-A-CATH care access/de-access, Wound Care, Intravenous Care, Diabetes Management Support, CVAD, and Drain Care management.
  • Responsible for the day-to-day management and coordination of the services your staff will provide to the community.
  • Thrives on being a self-starter and can work well under minimal supervision.
  • Can work in a team but is proven to be reliable while working independently.
  • Is a problem solver at heart and when problems do arise, is able to find solutions and provide the best customer service possible.
Requirements:
  • Undergraduate degree in Nursing from a recognized accredited school of nursing.
  • Active registration and in good standing with the College of Nurses of Ontario, as a Registered Nurse.
  • 5 years of clinical nursing experience, including 2 years in a supervisory or leadership role.
  • Highly energetic and enthusiastic individual who loves to work in a dynamic and fast-paced environment.
  • Knowledge of home care delivery, gerontology, and integrated care models.
  • Highly organized, with ability to multitask conflicting priorities.
  • Demonstrated leadership, clinical, and communication skills.
