Key Responsibilities
The successful candidate will serve as a centralized point of access connecting Primary Care Providers (PCPs) with appropriate community, clinical, and specialized supports for complex patients across the healthcare continuum. Key responsibilities include:
  • Serve as a single point of access to connect PCPs with relevant resources and services
  • Use assessment skills to collect patient information required for supporting care coordination
  • Triage calls appropriately by understanding the difference between emergent and non-emergent cases
  • Navigate different clinics and departments to support PCPs in managing medical, functional, cognitive, and psychosocial conditions
  • Enhance workflow and communication pathways between hospital, primary care, and community care by identifying gaps in care and promoting seamless pathways that optimize patient care

Qualifications
The ideal candidate will possess a regulated Allied health professional designation in good standing and at least 3 years of experience in acute care or community settings. Computer proficiency and ability to work remotely are also essential. A strong knowledge of hospital/community resources, relationship management, problem-solving, and critical thinking skills are also required.
