Job Summary

We are seeking a highly skilled Clinical Social Worker to join our team at Fraser Health. As a Master's level social worker, you will provide comprehensive social work services and therapeutic clinical counseling to patients and families in our assigned clinical specialty area.

Key Responsibilities
  • Conduct comprehensive social work and psychosocial assessments of patients and families, using a variety of assessment tools and in accordance with professional standards, clinical policies and practices.
  • Provide comprehensive therapeutic clinical services, including grief and bereavement counseling, to patients and families in the assigned clinical specialty area.
  • Provide consultation and education to patients, families, and other healthcare professionals on issues related to high-risk factors, recovery, and current practice in the field of adaptation.
  • Maintain patient records, documenting patient assessments, charting information, and preparing progress notes and reports in accordance with established standards, policies, and procedures.
  • Provide leadership to the interdisciplinary team in patient care planning, including discharge planning and hospital stay.
Qualifications
  • Master's degree in Social Work from an approved school of Social Work.
  • Current full registration with the BC College of Social Workers.
  • Comprehensive knowledge and understanding of emotional and mental health issues related to illness, injury, and patient and family cultural dynamics.
  • Comprehensive counseling skills, including crisis counseling, brief therapeutic interventions, and loss and grief applicable to patient, families, and their support networks.
