Job Title: Social Worker

At Southern Health-Santé Sud, we are seeking a highly skilled and compassionate Social Worker to join our team. As a Social Worker, you will play a vital role in improving the health and well-being of our clients within the My Health Team community.

Key Responsibilities:
  • Provide clinical assessment, treatment planning, and psychosocial counseling services to clients.
  • Collaborate with the My Health Team to navigate and manage client care.
  • Develop and implement education programs to promote optimal health outcomes.
  • Participate in program and community development to enhance service delivery.
  • Contribute to the planning and implementation of clinical social worker experiences for the Family Practice Medicine Residency Program.
Requirements:
  • Bachelor Degree in Social Work (BSW) from a Canadian Association for Social Work Education (CASWE) accredited school/faculty.
  • Registration with the Manitoba College of Social Workers.
  • Knowledge of health and social welfare systems.
  • Knowledge and understanding of cultural and spiritual sensitivity.
  • Two (2) years of recent and relevant health care experience in the last five years is preferred.
  • Demonstrated excellence in communication, interpersonal, and organizational skills.
  • Demonstrated ability to provide full Psychosocial Assessment and evidence-informed counseling interventions.
  • Demonstrated ability to work in a culturally informed manner within a range of cultures.
  • Commitment to the profession of Social Work and the Canadian Association of Social Work (CASW) Code of Ethics and the Manitoba College of Social Workers Standards of Practice.
Conditions of Employment:
  • Completes and maintains a satisfactory Criminal Record Check, Vulnerable Sector Search, Adult Abuse Registry Check, and Child Abuse Registry Check, as appropriate.
  • All Health Care workers are required to be immunized as a condition of employment in accordance with Southern Health-Santé Sud policy.
