Job Summary

We are seeking a skilled Social Worker to join our team at Southern Health-Santé Sud. As a Social Worker, you will play a vital role in improving the health and well-being of our clients within the My Health Team community.

Key Responsibilities
  • Provide clinical assessment, treatment planning, and psychosocial counseling services to clients
  • Collaborate with the My Health Team to develop and implement effective care plans
  • Provide education and support to clients to achieve optimal health outcomes
  • Participate in program and community development to enhance service delivery
  • Contribute to the Family Practice Medicine Residency Program by sharing clinical social worker experiences
Qualifications
  • Bachelor Degree in Social Work (BSW) from a Canadian Association for Social Work Education (CASWE) accredited school/faculty, or a provincially authorized post-secondary program, or the approved equivalent if internationally educated
  • Registration with the Manitoba College of Social Workers
  • Knowledge of health and social welfare systems, cultural and spiritual sensitivity, and ages/stages of life cycles
  • Understanding of right to choice/life/death issues, providing support and advice to patients/clients and families
  • Familiarity with community resources, supports, and services typically used by healthcare clients
  • Knowledge of policies, legislation, programs, and issues related to area of practice
  • Two (2) years of recent and relevant healthcare experience in the last five years is preferred
Conditions of Employment
  • Completes and maintains a satisfactory Criminal Record Check, Vulnerable Sector Search, Adult Abuse Registry Check, and Child Abuse Registry Check, as appropriate
  • All Health Care workers are required to be immunized as a condition of employment in accordance with Southern Health-Santé Sud policy
