About the Role

We are seeking a highly motivated and compassionate Community Residential Worker (CRW) to join our team at Metro Community Living. As a CRW, you will play a critical role in supporting individuals with physical and intellectual disabilities, as well as mental health challenges, to live their best lives in Nova Scotian communities.

In this role, you will provide direct support to participants, develop and implement individualized support plans, and collaborate with participants' treatment teams. You will also be responsible for daily planning, organization, and administrative functions related to service delivery.

Key Responsibilities:

1. Provide direct participant support and maintain confidentiality on all matters pertaining to the participants served.
2. Collaborate with the PC to conduct formal and informal participant assessments, complete Individual Program Plans (IPP), and quarterly reports.
3. Work collaboratively with participants' treatment teams, foster positive working relationships with community partners, and identify and document participant concerns.
4. Ensure participant living environments meet MCL's program standards and report any concerns to the supervisor.

Administrative Functions:

1. Ensure all participant cash request documentation is submitted to the supervisor monthly.
2. Follow standard operating procedures for all participant documentation, such as Daily Records, Appointment Record Forms, Incident Reports, Mileage Reimbursement Forms.

Qualifications:

To be successful in this role, you will have:

1. Education and a minimum of 1 year's experience in the Human Services sector.
2. Valid NS Driver's License and access to a reliable vehicle.
3. Strong communication skills, written and oral.
4. Experience in recreation and leisure and/or community services.
5. Clear and current criminal record check with vulnerable sectors.
6. Ability to travel the majority of the time.
