Clinical Team Leader

We are seeking a highly skilled Clinical Team Leader to join our team at Nova Scotia Health and IWK Health. As a key member of our interdisciplinary team, you will be responsible for providing leadership and innovation in the provision of collaborative, person-centred care to patients with a focus on functional health.
The successful candidate will work closely with patients and families, team members, community organizations, and primary, secondary, and tertiary health services to promote person-centered care within the Primary Health Care portfolio. You will develop and foster a healthy interdisciplinary team environment, building relationships with community-based services and ensuring that practice is interdisciplinary and evidence-based.

- Leadership and Innovation: Provide leadership and innovation in the provision of collaborative, person-centred care to patients with a focus on functional health.
