Job Title: Family Medicine Physician

We are seeking a part-time Family Medicine Physician to join our Integrated Chronic Care Services team. This is a unique opportunity to work in a collaborative and interdisciplinary environment, providing comprehensive care to individuals with complex chronic conditions.

About the Role

As a Family Medicine Physician, you will be responsible for providing 70% clinical care and 30% research activities. You will work closely with a team of healthcare professionals, including occupational therapists, nurse practitioners, registered nurses, and clinical therapists, to provide holistic care to patients.

Key Responsibilities
  • Provide comprehensive assessments and care planning for individuals with complex chronic conditions
  • Work collaboratively with the healthcare team to develop and implement individualized care plans
  • Conduct regular office hours to support team-based care and continuity of care
  • Participate in research activities to improve patient outcomes and advance the field of chronic care
