Physician - Chronic Care Specialist

We are seeking a part-time physician to join our Integrated Chronic Care Services team. As a key member of our interdisciplinary team, you will provide comprehensive assessments and care planning for individuals with complex chronic conditions.
About the Role:- Provide 70% clinical care and 30% research activities
- Collaborate with a team of health professionals, including physicians, occupational therapists, and registered nurses
- Focus on self-management support and addressing the needs of the whole person
- Participate in research activities to improve chronic disease outcomes
