About the Role

Vancouver Coastal Health is seeking a skilled Registered Nurse or Registered Psychiatric Nurse to join our team at Little Mountain Place in Vancouver, BC.

Key Responsibilities
  • Provide high-quality resident care in a Long Term Care setting, utilizing sound judgment and critical thinking skills.
  • Lead and coordinate clinical interventions, evaluating resident outcomes based on goals of care.
  • Act as a resource for the interdisciplinary team, providing information and coordination of ongoing resident care.
  • Support RCA's and LPN's, promoting cohesive team work and resident-centered care.
  • Enhance quality of life for residents and their families.
  • Consult, confer, and collaborate with other healthcare providers.
  • Maintain and advance clinical competence.
  • Participate in designated committees and research projects as assigned.
Requirements
  • Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the BC College of Nurses and Midwives.
  • Completion of an approved gerontology course or equivalent combination of education, training, and experience.
  • Demonstrated ability in nursing practice related to residents with chronic diseases.
  • Demonstrated knowledge of pathophysiology, aging changes, polypharmacy, treatment protocols, and nursing interventions.
  • Ability to provide nursing assessments and interventions related to common diagnoses and specific care situations.
  • Ability to recognize physical adaptations required for frail elderly and/or physically impaired residents.
  • Ability in End of Life care approaches, Embedding Palliative Approach in Care (EPAC) in long term care.
  • Ability in Chronic Disease Management, anticipating and responding to potential complications and care issues.
  • Correct usage of equipment such as lifts, oxygen, suction machines, pumps, and other related equipment.
  • Ability to develop care plans and implement care in relation to resident and family priorities.
  • Assessment and management of residents with acute/chronic pain management, including those with cognitive impairment.
  • Safe care and management of residents with Oxygen Therapy, VAC Dressing, Wound Care, Catheters, Ostomy Care, Tracheostomy Care, Ventilator Management, and other treatments.
  • Performing RN autonomous scope of practice and standards of care relevant to designated patient/client population.
