About the Role

This is a full-time position that requires flexibility in scheduling, including weekends and on-call work. As a nurse in our community setting, you will have the opportunity to develop meaningful therapeutic relationships with patients, preserve patient dignity and privacy, and exercise your full scope of practice.

Responsibilities

As a nurse in our community setting, your responsibilities will include:

  • Developing and maintaining effective care plans with respect to community health services
  • Providing exceptional nursing services to patients in their homes
  • Communicating and liaising with patients, family members, and other members of the interdisciplinary care team
  • Performing other clinical duties as required
Requirements

To be successful in this role, you will need:

  • Current College of Nurses of Ontario (CNO) registration in good standing
  • A strong desire and commitment to making a difference in the lives of our patients
  • Exceptional interpersonal skills
  • A valid driver's license and vehicle
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record including 2 step TB test
  • To protect our patients, we require a current vulnerable sector check and a clear background check
