Job Summary

CarePartners is seeking a skilled and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as a Visiting Nurse in the Burks' Falls, ON area. This part-time position offers flexible scheduling and the opportunity to work in a supportive and collaborative environment focused on delivering high-quality patient care.

Key Responsibilities

  • Develop and maintain effective care plans for patients receiving community health services
  • Provide exceptional nursing services to patients in their homes, both in-person and virtually
  • Communicate and liaise with patients, family members, and other healthcare professionals to ensure seamless care
  • Perform other clinical duties as required

Requirements

  • Current College of Nurses of Ontario (CNO) registration in good standing
  • Strong desire and commitment to making a difference in the lives of patients
  • Exceptional interpersonal skills
  • Valid driver's license and vehicle for regular travel
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record, including 2-step TB test
  • Vulnerable sector check and clear background check
