Nurse Care Specialist

As a Registered Nurse/Registered Psychiatric Nurse with Fraser Health, you will play a vital role in providing person-centered care to diverse populations. This includes promoting resident/client choices, individualized care plans, and supporting continuity and consistency of care.
Key Responsibilities- Collaborate with the interdisciplinary team to identify current or potential care concerns and strengths, plan interventions, and evaluate outcomes.
- Provide direct nursing care, including wound care, dementia care, and palliative care, and develop care priorities in accordance with established policies and standards of nursing practice.
- Communicate resident/client assessments and changes to Physicians, Resident/Client Care Coordinator, and the interdisciplinary team.
- Participate in care planning, supporting decision making and person-centered care, and act as an advocate to protect and promote residents'/client's rights.
- Work in partnership with Licensed Practical Nurses and other interdisciplinary team members to share information, demonstrate and/or provide guidance on clinical tasks, and participate in quality improvement activities.
- Graduate of an approved School of Nursing or Psychiatric Nursing.
- Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- One year's recent, related gerontological nursing experience or an equivalent combination of education, training, and experience.
- Knowledge of nursing theory and evidence-informed practice.
- Ability to apply clinical knowledge in practice, apply critical thinking skills, problem solve, and effectively resolve conflict situations.
- Ability to communicate effectively both verbally and in writing, provide leadership and clinical guidance, and work independently and as a member of the interdisciplinary team.
