Key Responsibilities- Conduct comprehensive assessments and develop individualized care plans to meet the unique needs of clients with complex chronic conditions.
- Provide direct client care, including health counseling, education, and self-management support to promote optimal health outcomes.
- Collaborate with the interprofessional team, primary care providers, and community resources to coordinate care and services, ensuring a seamless transition across the healthcare continuum.
- Facilitate care conferencing to review client care plans, identify areas for improvement, and develop strategies to enhance patient outcomes.
- Develop and implement quality improvement initiatives to enhance clinical practice, healthcare services, and patient satisfaction.
- Participate in departmental quality improvement and risk management activities, identifying areas for improvement and contributing to data collection and analysis.
- Provide input in the development and revision of standards of care, policies, and procedures, advocating for improvements in clinical practice and healthcare services.
- Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- One year's recent related clinical experience assessing and treating complex geriatric and/or adult patients with chronic health conditions in an acute or community/outpatient care setting.
- Valid BC Driver's license and access to a personal vehicle for business-related purposes.
- Demonstrated knowledge, skills, and competence in gerontology and adults living with complex frailty and chronic illnesses.
- Ability to communicate effectively, both verbally and in writing.
- Ability to integrate and evaluate pertinent data from multiple sources to problem-solve effectively.
- Knowledge of broad healthcare services, community resources, and their role in providing a continuum of care.
