Job Summary:

As a Personal Support Worker at VON Canada, you will play a vital role in providing companionship, personal care, and health maintenance services to clients in their homes or communities. You will work collaboratively with the healthcare and personal support teams to ensure clients receive the highest level of independence and well-being.

Key Responsibilities:

  • Conduct thorough assessments of clients' homes, personal situations, and care plans to identify areas for improvement and ensure their needs are met.
  • Develop and implement personalized care plans in collaboration with the Personal Support Program Manager and other healthcare professionals.
  • Provide personal care services, including light housekeeping, meal preparation, and assistance with daily living activities.
  • Monitor and maintain household safety, and report any concerns to the supervisor or Personal Support Program Manager.
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
  • Participate in multidisciplinary team meetings to discuss client care plans and share best practices.
  • Adhere to strict confidentiality and maintain professional boundaries at all times.
  • Perform other duties as assigned by the supervisor or Personal Support Program Manager.

Requirements:

  • High school diploma or equivalent.
  • CCA Certification (Nova Scotia) or equivalent.
  • Personal Support Worker (PSW) certification or equivalent.
  • Standard First Aid Certificate or CPR level C Certificate (if applicable).
  • Experience working with elderly or ill individuals or children.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work varied hours, including evenings and weekends.
