About the Role
As a Nurse Team Lead, you will be responsible for providing exceptional care and support to our clients in their homes. You will lead a team of caregivers, providing guidance, mentorship, and coaching to ensure they deliver the highest quality care. You will also be responsible for building relationships with clients and their families, understanding their unique needs, and developing personalized care plans.
Key Responsibilities
- Supervise and support a team of caregivers to ensure they provide high-quality care and adhere to Bayshore's standards and policies.
- Conduct regular home visits to assess client needs, provide care, and ensure the quality of care is maintained.
- Develop and implement care plans that meet the unique needs of each client.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Provide training and mentorship to caregivers to enhance their skills and knowledge.
Requirements
- Current registration in good standing with the provincial nursing regulatory body.
- Minimum 5 years of experience in community nursing, with a focus on leadership and team management.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, caregivers, and other healthcare professionals.
- Ability to work independently and as part of a team, with a focus on delivering high-quality care and achieving exceptional results.
