At Northern Health Authority, we are seeking a skilled and compassionate Primary Care LPN to join our team. As a member of our interprofessional team, you will provide high-quality, patient-centered care to individuals and families in our communities.
About the Role
This is an exciting opportunity to work in a dynamic and supportive environment, where you will have the chance to grow your skills and make a meaningful difference in the lives of our patients. As a Primary Care LPN, you will be responsible for providing clinical follow-up, conducting assessments, and implementing care plans in collaboration with our interprofessional team. You will also have the opportunity to work with a variety of patient populations, including those with acute care, gerontology, and palliative needs.

Qualifications
To be successful in this role, you will require a diploma in Practical Nursing from a recognized program, as well as one year of recent, related experience. You must also be registered with the BC College of Nursing Professionals as a practicing LPN registrant and hold a current, valid BC Driver's License. You will also need to demonstrate strong communication and interpersonal skills, as well as the ability to work independently and collaboratively as part of an interprofessional team.

• Demonstrated knowledge and skills within the LPN defined scope of practice or patient population dependent on the nature of the caseload.
• Ability to communicate effectively, both verbally and in writing.
• Ability to establish and maintain rapport with people and their families.
• Ability to work independently and collaboratively with others within the context of an interprofessional team.
• Ability to observe and recognize changes in a person's condition or health status.
• Ability to plan, organize and prioritize; and ability to implement a plan of care.
• Ability to operate related equipment including proficiency in relevant computer applications.
• Physical ability to perform the duties of the position.
