Job Summary

We are seeking a highly skilled and compassionate Bilingual Nurse Case Manager to join our team at McKesson. As a key member of our patient support team, you will play a vital role in ensuring that patients receive seamless and comprehensive support throughout their healthcare journey.

Key Responsibilities
  • Patient Support
    • Provide personalized support to patients and their families, addressing their unique needs and concerns
    • Collaborate with healthcare providers to ensure timely and effective care coordination
    • Develop and implement individualized care plans to optimize patient outcomes
  • Stakeholder Interactions
    • Build and maintain strong relationships with healthcare providers, pharmacies, and other stakeholders to ensure seamless care coordination
    • Communicate effectively with patients, families, and healthcare teams to ensure clear understanding of care plans and progress
    • Identify and address potential barriers to care, developing creative solutions to ensure patient access to necessary services
  • Key Requirements
    • Communication & Soft Skills
      • Excellent verbal and written communication skills, with ability to communicate effectively with diverse populations
      • Strong interpersonal and leadership skills, with ability to build and maintain strong relationships with stakeholders
    • Technical & Business Understanding
      • Strong computer and software skills, with ability to navigate electronic health records and other healthcare systems
      • Knowledge of healthcare policies, procedures, and regulations, with ability to apply this knowledge to ensure compliance and optimal patient outcomes
    Requirements
    • 5-7 years of experience in case management, patient advocacy, or a related field
    • Strong analytical and problem-solving skills, with ability to think critically and develop creative solutions
    • Excellent time management and organizational skills, with ability to prioritize tasks and manage multiple projects simultaneously
