Job Summary

We are seeking a highly skilled Public Health Substance Use Outreach Specialist to join our team at Fraser Health. As a key member of our interprofessional care team, you will provide essential support and services to individuals who use substances, promoting harm reduction, health promotion, and prevention strategies.

About the Role
  • Provide outreach nursing care to individuals in their home setting, primary care setting, or suitable alternative, including those who are homeless or at risk of homelessness.
  • Conduct initial and ongoing mental health and substance use client assessments, providing technical, therapeutic, and safety interventions as needed.
  • Work collaboratively with individuals, families, and communities to develop and implement comprehensive care plans that address individual needs and goals.
  • Collaborate with healthcare staff, social services, and community agencies to ensure seamless access to services and supports.
Key Responsibilities
  • Care Coordination: Coordinate care for individuals by collaborating with the interprofessional care team, performing intake functions, conducting substance use and mental health assessments as required, developing and implementing comprehensive care plans based on individual identified needs and best practices.
  • Harm Reduction Education and Supplies: Provide harm reduction education, counseling, supplies, and support, including episodic overdose prevention services (eOPS) and drug checking in partnership with other team members.
  • Social Determinants of Health: Identify the health issues, behaviors, needs, barriers, and challenges associated with activities and risks within the framework of health promotion, harm reduction, and prevention.
  • Crisis Intervention and Counseling: Engage individuals and their families in learning about substance use, evidence-based practices, and supports; facilitate connections to community resources and services; and provide crisis intervention, individual counseling, and education according to various theoretical models.
  • Documentation and Reporting: Prepare and maintain accurate records in accordance with established standards and procedures, adhering to practice standards and Fraser Health protocols.
Qualifications
  • Education and Experience: Graduation from an approved School of Nursing or Psychiatric Nursing, two years recent related experience providing services to individuals who use substances, or an equivalent combination of education, training, and experience.
  • Licenses and Certifications: Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM); valid Class 5 BC Driver's License and access to personal vehicle with appropriate insurance coverage.
Estimated Salary

$83,000 - $115,000 per year, depending on experience and qualifications.

This salary estimate is based on data from the Canadian Institute for Health Information (CIHI), the Government of British Columbia, and job postings from similar organizations in the region.
