Care Delivery Manager

The Patient Care Coordinator plays a pivotal role in providing clinical supervision, guidance, and support to nursing staff and other clinical team members within the assigned unit(s). This position is responsible for the day-to-day operations of the assigned unit(s), including daily patient flow, and supports/participates in research, policy, quality improvement, and risk management activities.
Key Responsibilities- Applies Established Criteria: Prioritizes admissions/intake, leads and facilitates actions related to daily patient flow, plans for patient discharge, and appropriate patient placement/referral by ensuring the care team follows through on specific care paths and processes.
- Supervises and Coordinates: Supervises nursing and nursing support staff and coordinates the work of the clinical team overall on a day-to-day basis, observing work and correcting where necessary, monitoring the quality of care and the safety of patients.
- Supports Quality Patient Care: Promotes interprofessional collaboration and team building, supports staff to plan and carry out patient/family education regarding illness prevention and health maintenance, and acts as a mentor and resource to staff.
- Initiates and Leads: Initiates/leads/supports plan of care conferences and rounds for the purpose of optimal patient care management according to best practice; disseminates clinical information to nursing/nursing support staff and other members of the clinical team.
- Provides Input and Support: Identifies learning requirements of staff and, in conjunction with the Manager, Educator, and other members of the clinical team, provides input into the development of learning objectives; provides clinical support and guidance and evaluates the effectiveness of learning opportunities.
- Acts as a Point Person: Acts as a point person for patients and families who have concerns or questions regarding their care; follows up as required.
Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Three years of recent related clinical experience in the designated clinical area, including one year of supervisory experience, or an equivalent combination of education, training, and experience.
Completion of postgraduate coursework and/or certification applicable to the designated clinical area.
