Resident Care Coordinator

About the Role
The Resident Care Coordinator will be responsible for providing high-quality nursing care to residents in a Long Term Care setting. This role requires a strong clinical background, effective leadership skills, and the ability to work collaboratively with interdisciplinary teams.
Key Responsibilities
- Develop and implement individualized care plans for residents, ensuring their physical, emotional, and social needs are met.
- Supervise and mentor nursing staff, providing guidance and support to ensure high-quality care is delivered.
- Collaborate with the interdisciplinary team to ensure seamless care transitions and effective communication.
- Conduct regular assessments of residents' needs, identifying areas for improvement and implementing changes as needed.
- Ensure accurate and timely documentation of resident care, including medication administration and medical records.
- Manage nursing supplies and equipment, ensuring adequate stock levels and efficient use of resources.
- Communicate effectively with residents, families, and healthcare providers to ensure informed decision-making and care coordination.
- Participate in ongoing training and professional development to stay current with best practices and industry standards.
- Perform other duties as required to support the delivery of high-quality resident care.
Requirements
- Current registration as a Registered Nurse with the College of Registered Nurses of Alberta (CARNA).
- Current CPR (BCLS) certification.
- Proven leadership and clinical skills, with experience in Long Term Care or Support Living settings.
- Effective communication and interpersonal skills, with the ability to work collaboratively with diverse teams.
- Flexibility to work rotating shifts, including days, evenings, nights, and weekends.
- Ability to communicate in Chinese is an asset.
