Licensed Practical Nurse

At West Coast Seniors Housing Management, we are seeking a skilled Licensed Practical Nurse to join our care team as a leader and coordinator. This role is responsible for coordinating care delivery, performing duties, and monitoring departmental activities in accordance with their scope of practice, established standards, regulations, policies, and procedures to maximize individualized and resident-centered care.
Key Responsibilities:
- Assess residents' needs, implement, and evaluate individualized resident-centered care plans.
- Ensure residents' rights are observed and adhere to a holistic philosophy of wellness.
- Liaise with physicians, community agency representatives, families, visitors, volunteers, other disciplines, and departments to ensure quality resident-centered care is delivered.
- Assess and treat all wounds on the unit and act as a resource to care staff for strategies to prevent skin breakdown.
- Administer prescribed medications and treatments within the limitations of the BC College of Nurses and Midwives (BCCNM).
- Monitor and report changes in physical and psychological conditions of residents, document changes on the resident health record, notify family and physician of changes in condition.
- Active participant in Interdisciplinary Care Conferences.
- Participate in care plan reviews to ensure current and individualized care plans.
- Support administrative requirements at the community in accordance with established standards, including but not limited to; admissions, discharge, and ensuring confidentiality is maintained at all times.
- Actively participate in orientation of new staff, community programs, meetings, committees, and educational in-services.
- Provide coaching and direction to care staff as needed and use in-the-moment education to improve resident care, which includes completing all Point of Care (POC) documentation. Seek assistance/support from RN or designate as needed.
- Maintain an up-to-date knowledge of community policies and procedures, particularly those concerning emergency procedures, including equipment management, guidance, and assistance on safe work practices.
- Perform other related duties as required.
Qualifications and Skills:
- Graduate of an approved program for Practical Nursing and holds a current license with the British Columbia College of Nurses and Midwives (BCCNM) as an LPN.
- Valid Occupational First Aid Level 1 certification.
- Successful annual Resident Assessment Instrument (RAI) competency testing.
- Previous experience in a long-term care facility is an asset.
- All successful applicants must pass the vulnerable sector Criminal Record Check applicable to Provincial guidelines (see BCCNM license).
- Ability to read, write, and speak English proficiently and understand verbal and written instructions.
- Computer skills, including the ability to navigate basic computer programs such as Word, Excel, Outlook.
- Ability to push, pull, stoop, crouch, lift, and carry up to 25 lbs. Must be able to work with cleaners, detergents, and other cleaning equipment.
