Home Care Clinical Lead

We are seeking a dedicated and compassionate Home Care Clinical Lead to join our team at SE Health. In this critical role, you will provide comprehensive care in an interdisciplinary setting, working closely with clients, families, and caregivers to deliver person-centered care.
Key Responsibilities- Primary Care Provider: Serve as the primary contact for clients, providing continuous and comprehensive support to their care teams.
- Care Planning: Conduct client assessments, develop interdisciplinary care plans based on best evidence and client needs, and lead the care planning process.
- Communication: Facilitate regular interdisciplinary team communication, ensuring seamless coordination of care.
- Discharge and Transition Planning: Lead discharge and transition care planning discussions, collaborating with front-line staff to ensure smooth transitions.
- Family and Caregiver Support: Facilitate family and caregiver meetings, supporting clients through care transitions by connecting them with community resources.
- Nursing Experience: Minimum of two years of nursing experience, preferably in home/community care or acute care settings.
- Clinical Leadership: Demonstrated clinical leadership experience, with excellent assessment skills and a strong clinical background.
- Interpersonal Skills: Excellent interpersonal, communication (oral and written), and critical thinking skills, with the ability to work independently and collaboratively within a team environment.
- Education and Certification: Registered in good standing with the College of Nurses of Ontario, with a vulnerable sector check (current).
