Nurse Practitioner Community Health Specialist

As a Community Health Nurse with Fraser Health Authority, you will play a vital role in providing comprehensive nursing care to clients in their homes and communities.
Job SummaryWe are seeking a skilled and compassionate Registered Nurse to join our team as a Community Health Nurse. In this role, you will work independently and as part of an interprofessional team to assess, plan, implement, and evaluate client care plans. Your expertise will be essential in promoting health, preventing illness, and supporting clients and their families to live confidently and safely at home and/or in the community.
Key Responsibilities- Conduct initial and ongoing assessments of clients' physical, emotional, and social needs.
- Develop, implement, and evaluate individualized client care plans that promote health, prevent illness, and support clients in achieving optimal levels of function.
- Collaborate with clients, families, caregivers, healthcare providers, and other professionals to ensure seamless transitions across the healthcare continuum.
- Provide direct client care, including education, counseling, and interventions, to promote health and well-being.
- Maintain accurate and detailed clinical records, including intake screens, client assessments, care plans, and progress notes.
- Degree in Nursing from an accredited institution.
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- One year of recent experience in a community or outpatient setting, preferably working with geriatric and adult populations with chronic health conditions.
- Demonstrated knowledge of gerontology and adults living with complex frailty and chronic illnesses.
- Able to complete initial and ongoing client assessments and provide nursing care through therapeutic interventions.
- Effective communication and interpersonal skills, both verbally and in writing.
- Ability to integrate and evaluate pertinent data from multiple sources to problem-solve effectively.
This is a full-time position working 37.5 hours per week. You will be required to travel throughout the community using your personal vehicle for business-related purposes.
Salary and BenefitsThe salary range for this position is approximately $52-$65 per hour, based on qualifications and experience. Fraser Health Authority offers a comprehensive benefits package, including four weeks of vacation, employer-paid health and dental benefits, pension plan, and more.
