Job Summary

We are seeking a skilled Licensed Practical Nurse to join our hemodialysis team at Fraser Health. As a key member of our interdisciplinary team, you will provide high-quality patient care and contribute to the development and implementation of individualized care plans.

Key Responsibilities
  • Assess, plan, implement, and evaluate patient care during the pre-dialysis, intra-dialysis, and post-dialysis periods.
  • Establish and maintain therapeutic relationships with patients and their families through effective communication and interpersonal skills.
  • Contribute to multidisciplinary care planning meetings and family conferences to ensure comprehensive care.
  • Advocate for patient interests and preferences in care planning and decision-making.
  • Collaborate with registered nurses and other healthcare professionals to provide seamless patient care.
  • Perform nursing duties related to hemodialysis, including patient assessment, medication administration, and vital sign monitoring.
  • Document patient information and updates in a timely and accurate manner.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • Grade 12 diploma and completion of a Practical Nursing program.
  • Current full practicing registration with the British Columbia College of Nurses and Midwives.
  • One year of recent experience working as a Licensed Practical Nurse in acute care or a related field.
  • Current certificates in CPR Level C and First Aid.
