We are seeking a highly motivated and compassionate Social Services Worker to join our team at Prokare Support Services Inc. As a key member of our team, you will play a vital role in providing support and services to individuals and families in need.
Key Responsibilities- Case Management: Assess clients' needs and develop personalized plans to address their challenges.
- Support Services: Provide emotional support, guidance, and resources to clients to help them achieve their goals.
- Program Development: Collaborate with our team to design and implement programs that meet the needs of our clients.
- Community Outreach: Establish relationships with community partners and organizations to provide referrals and support services.
- Documentation: Maintain accurate and detailed records of client interactions and program progress.
- Education: College/CEGEP diploma or equivalent.
- Experience: 1 year to less than 2 years of experience in a related field.
- Language: Fluency in English.
- Work Hours: 30 to 44 hours per week.
