Social Services Professional

We are seeking a highly skilled and compassionate Social Services Worker to join our team at Prokare Support Services Inc. As a key member of our team, you will play a vital role in providing support and services to our clients.
Key Responsibilities- Case Management: Assess clients' needs and develop personalized plans to address their challenges.
- Service Coordination: Liaise with other social services agencies and healthcare providers to ensure seamless support for our clients.
- Program Development: Collaborate with our team to design and implement life skills workshops, substance abuse treatment programs, and behavior management programs.
- Counseling and Support: Provide individual and group counseling sessions to clients, as well as accompany them to appointments and social outings.
- Registry Management: Maintain a registry of special needs support workers and ensure that our clients receive the highest level of care.
- Program Evaluation: Assist in evaluating the effectiveness of our treatment programs and make recommendations for improvement.
- Client Support: Assist clients with special needs and provide general support to ensure their well-being.
- Education: College/CEGEP diploma or equivalent.
- Experience: 1 year to less than 2 years of experience in a related field.
- Language: Fluency in English.
- Work Hours: 30 to 44 hours per week.
