About the Role
Circle of Care is seeking a skilled and compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in providing high-quality support services to Holocaust Survivor clients, ensuring their needs are met and their well-being is prioritized.
Key Responsibilities
- Respond to client inquiries and provide guidance on the application process, ensuring a smooth and efficient experience.
- Conduct in-home and in-office visits to support client applications, building trust and rapport with clients and their caregivers.
- Assist clients in collecting necessary information and documents, facilitating a seamless application process.
- Develop and implement care plans in collaboration with clients and caregivers, ensuring their unique needs are addressed.
- Provide counseling and informal support as needed, promoting client well-being and independence.
- Collaborate with the Holocaust Survivor Services team and care providers to ensure a comprehensive and coordinated approach to client care.
Requirements
- A degree in social work, nursing, or a related field, with a strong understanding of community health providers and home care options.
- Minimum 1-3 years of clinical experience, preferably in community care, home care, or case management.
- Experience working with geriatric populations, Holocaust Survivors, and/or survivors of trauma is highly valued.
- Excellent organizational, communication, and interpersonal skills, with the ability to work independently and as part of a dynamic team.
- Strong advocacy skills, patient-centered focus, and decision-making abilities are essential.
