About the Role:
We are seeking a highly skilled and experienced Wound Care Specialist to join our team at ParaMed Home Health Care. As a key member of our nursing team, you will provide expert care and management to patients requiring wound, ostomy, and continence assessment and treatment.
Key Responsibilities:
- Provide direct practice to patients requiring wound, ostomy, and continence assessment, care, and management.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with internal and external stakeholders to ensure seamless care coordination.
- Participate in wound, ostomy, and continence clinical leadership opportunities to advance best practices.
- Precept new nurses and provide education to the nursing team on wound, ostomy, and continence care.
Requirements:
- Current Certification as a Nurse Specialized in Wound Ostomy & Continence (NSWOC).
- Minimum of 2 years' experience as a practicing Registered Nurse, ideally in a community setting and/or providing wound, ostomy, and continence care to patients.
- Current membership/registration with the Nurses Specialized in Wound Ostomy & Continence Canada (preferred).
- A reliable vehicle is a must.
