Patient Care Coordinator

We are seeking a compassionate and skilled Registered Practical Nurse (RPN) to join our healthcare team in Newmarket, Canada. In this role, you will provide high-quality nursing care to patients in various settings.
Key Responsibilities:- Assess patients' needs and develop individualized care plans, taking into account their unique circumstances.
- Administer medications and treatments as prescribed by physicians, ensuring accurate dosages and administration.
- Monitor patients' vital signs and report any changes in condition to the healthcare team.
- Collaborate with healthcare professionals to ensure coordinated care and optimal patient outcomes.
- Provide education and support to patients and their families, promoting health literacy and empowerment.
- Valid RPN license in Canada.
- Completion of a recognized Practical Nursing program.
- Previous experience in a healthcare setting is preferred.
- Strong communication and interpersonal skills.
- Ability to work in a fast-paced environment.
