Community Support Specialist

About the Role:
Fitzii is seeking a highly skilled Community Support Specialist to join our team. As a key member of our team, you will provide essential support services and guidance to adults with dual diagnosis in a community-based setting.
Key Responsibilities:
- Develop and implement personalized life plans for individuals with dual diagnosis, focusing on their strengths and abilities.
- Collaborate with support teams to ensure seamless delivery of services and supports tailored to each individual's needs.
- Provide coaching and guidance to help individuals make informed decisions and build essential life skills.
- Manage and maintain accurate client information and documentation, adhering to confidentiality and data protection guidelines.
- Participate in regular team meetings to share knowledge, best practices, and updates on individual progress.
Requirements:
- A post-secondary diploma or degree from a human services field, such as psychology, social work, or disability studies.
- Specialized education, training, or certification in developmental disabilities, behavior management, or related fields.
- Proof of completion of an approved Pharmacology course.
- Current First Aid and CPR certification.
- Experience working with adults with dual diagnosis in a community-based setting, preferably in a group living or independent living environment.
