We are seeking a highly skilled Nurse Practitioner to join our team at Homewood Health, a leading provider of medically focused, evidence-based treatment of mental health and addiction disorders.
Key Responsibilities- Provide comprehensive health assessments and diagnoses for patients with complex mental health needs
- Develop and implement individualized treatment plans in collaboration with the interdisciplinary team
- Work closely with consulting physicians to facilitate early assessment and intervention for patients requiring consults
- Support the management of patients through comprehensive nursing/medical history, physical and psychosocial assessment
- Utilize established care paths and order sets to guide care practices for the patient population
- 3+ years of experience as a Nurse Practitioner, with mental health and addiction expertise preferred
- Current registration in the Extended Class (Nurse Practitioner) with the College of Nurses of Ontario is required
- Active membership in related professional associations required, e.g. NPAO, RNAO
- Mental Health Nursing certificate preferred
- Masters of Nursing required
