About the Role

We are seeking a highly skilled Nurse Practitioner to join our team at Homewood Health, a leading provider of medically focused, evidence-based treatment of mental health and addiction disorders.

Key Responsibilities
  • Provide comprehensive health assessments and diagnoses for patients with complex mental health needs
  • Develop and implement individualized treatment plans in collaboration with the interdisciplinary team
  • Work closely with consulting physicians to facilitate early assessment and intervention for patients requiring consults
  • Support the management of patients through comprehensive nursing/medical history, physical and psychosocial assessment
  • Utilize established care paths and order sets to guide care practices for the patient population
Requirements
  • 3+ years of experience as a Nurse Practitioner, with mental health and addiction expertise preferred
  • Current registration in the Extended Class (Nurse Practitioner) with the College of Nurses of Ontario is required
  • Active membership in related professional associations required, e.g. NPAO, RNAO
  • Mental Health Nursing certificate preferred
  • Masters of Nursing required
