Homewood Health is seeking a skilled Nurse Practitioner to join our team at the Homewood Health Centre. As a key member of our interdisciplinary team, you will provide advanced practice nursing to patients and families with complex mental health needs.
Key Responsibilities:- Conduct comprehensive health assessments and develop individualized treatment plans
- Collaborate with consulting physicians and other healthcare professionals to provide high-quality patient care
- Support the management of patients with complex mental health needs, including those with substance use disorders
- Utilize established care paths and order sets to guide care practices
- Document patient information accurately and communicate effectively with the healthcare team
- 3+ years of experience as a Nurse Practitioner, with mental health and addiction expertise preferred
- Current registration in the Extended Class (Nurse Practitioner) with the College of Nurses of Ontario
- Active membership in related professional associations, such as NPAO and RNAO
- Mental Health Nursing certificate preferred
- Masters of Nursing required
