Job Title: Nurse Practitioner, Homewood Health Centre

Make a meaningful difference in the lives of individuals and families affected by mental health and addiction disorders. As a Nurse Practitioner at Homewood Health Centre, you will be part of a dynamic and collaborative team dedicated to delivering exceptional patient care.

Key Responsibilities:
  • Provide comprehensive health assessments and diagnoses for patients with complex mental health and addiction needs.
  • Develop and implement individualized treatment plans in collaboration with the interdisciplinary team.
  • Work closely with consulting physicians to facilitate early assessment and intervention for patients requiring consults.
  • Support the management of patients through comprehensive nursing and medical history, physical and psychosocial assessment, and established care paths.
  • Document patient information accurately and communicate effectively with the healthcare team.
Requirements:
  • 3+ years of experience as a Nurse Practitioner, with mental health and addiction expertise preferred.
  • Current registration in the Extended Class (Nurse Practitioner) with the College of Nurses of Ontario.
  • Active membership in related professional associations, such as NPAO and RNAO.
  • Mental Health Nursing certificate preferred.
  • Masters of Nursing required.
  • Innovative and professional role model with excellent assessment and clinical skills.
  • Ability to prioritize and organize multiple demands, work autonomously, and possess a working knowledge of the Mental Health Act and community mental health resources.
