Social Care Specialist

We are seeking a highly skilled Social Worker to join our team at Island Health. As a Social Care Specialist, you will be responsible for providing comprehensive social work services to patients and their families.
Estimated Salary: $65,000 - $85,000 per yearThis is a full-time position with a competitive salary range based on qualifications and experience.
Responsibilities- Provide assessment, planning, facilitation, care coordination, evaluation, and advocacy for patients and their families in a medical setting.
- Develop and implement individualized plans of care that address the unique needs of patients and their families.
- Maintain accurate and up-to-date records of patient interactions and progress.
- Collaborate with healthcare professionals and community resources to ensure seamless care delivery.
- Bachelor of Social Work degree (BSW) from an accredited school of social work or Master's degree (MSW) preferred.
- Current full registration with the BC College of Social Workers.
- At least 2 years of experience in a medical setting, preferably in social work.
- Demonstrated clinical competence and excellent communication skills.
