About the Role

As a Registered Nurse, you will play a vital role in providing comprehensive care to clients in their homes and communities. This is an exciting opportunity to work with a dynamic team that puts the client at the forefront of our care approach.

Key Responsibilities:

">
  • Provide continuous and comprehensive support to clients in their homes and communities;
  • Lead the care planning process and work with other disciplines to complete and implement interdisciplinary client care plans;
  • Facilitate regular communication with the interdisciplinary team to ensure seamless care delivery;
  • Support clients through care transitions by connecting them with community resources and services;
  • Develop strong self-care strategies with clients, families, and caregivers to promote health and wellbeing;

Requirements

To be successful in this role, you must have:

">
  • A Bachelor's Degree in Nursing or equivalent experience;
  • Canadian Nurses Association (CNA) community nursing certification or willingness to obtain;
  • Minimum of two years of nursing experience, preferably in home/community care, acute care, chronic disease management, health promotion, palliative care, or mental health;
  • Diverse nursing experiences working with different client groups;
  • Excellent skills in case management, coordination, and teamwork;
  • A valid driver's license and reliable transportation;
