Fraser Health is seeking a skilled and compassionate Indigenous Clinical Social Worker to join our team in Chilliwack. As a key member of our interdisciplinary team, you will provide comprehensive services and therapeutic clinical counseling to Indigenous clients and their families, working within a culturally sensitive and trauma-informed practice model.
Key Responsibilities:- Conduct comprehensive social work and psychosocial assessments of Indigenous clients and their families, using a variety of assessment tools to identify needs and develop effective treatment plans.
- Provide clinical support to the interdisciplinary team, including physicians, to develop care plans and services that meet the unique needs of Indigenous clients.
- Collaborate with community agencies and caregiver support groups to ensure seamless care and service delivery across the healthcare system.
- Participate in local committees and planning bodies to identify areas for improvement and enhance patient care services.
- Current full registration with the BC College of Social Workers.
- Two years' recent related experience in a complex healthcare environment, such as a community or home health setting, plus relevant related experience in an Indigenous environment/community.
- Valid BC Driver's License and access to a personal vehicle for business-related purposes.
- Comprehensive knowledge and understanding of counselling and clinical practice models as it relates to Indigenous individuals, families, groups, and communities.
- Demonstrated ability to conduct comprehensive social work and psychosocial assessments.
- Comprehensive counselling skills, including crisis counseling, brief therapeutic interventions, and loss and grief applicable to patients, families, and their support networks.
- Possesses a clear understanding of Trauma-Informed Practice within Indigenous Communities.
- Familiarity with traditional wellness practices, customs, values, and traditions of local First Nations.
- Comprehensive knowledge of pertinent/relevant healthcare and privacy legislations.
- Ability to educate and provide consultation to patients, families, and healthcare professionals.
- Ability to effectively work independently and with others, including working effectively in a multicultural environment, as part of an interdisciplinary team.
- Demonstrated ability to communicate effectively both verbally and in writing.
- Demonstrated ability to organize and prioritize workload in a complex environment.
- Ability to operate related equipment, including applicable software applications.
- Physical ability to carry out the duties of the position.
