We are seeking a highly skilled Care Coordination Specialist to join our dynamic healthcare team at CareOnDemand.

About the Role:

The successful candidate will be responsible for coordinating patient movement through our healthcare system, employing creative problem-solving skills to identify opportunities for process improvement in bed assignment, transfer, patient flow, and resource utilization.

This role requires attention to detail, the ability to work under pressure, and excellent communication skills. The ideal candidate will have 1-2 years of experience in a similar role and a strong understanding of healthcare logistics.

Key Responsibilities:
  • Coordinate patient care services with other healthcare providers to ensure seamless delivery of care.
  • Develop and implement process improvements to enhance patient flow and resource utilization.
  • Maintain accurate records and reports to track patient movement and care coordination activities.
  • Collaborate with healthcare teams to identify and address patient needs.
What We Offer:
